PRK in Brief

PRK (PhotoRefractive Keratectomy) is the surface ablation procedure where the corneal epithelium is removed to expose the stromal layer. The excimer laser then directly reshapes the corneal surface in approximately 5-30 seconds per eye (depending on diopter value): flattening the cornea for myopia or giving it a more symmetrical shape for astigmatism. Finally, a therapeutic contact lens is applied to protect the eye until epithelium regeneration (4-7 days).

Who Can Have PRK

  • People aged 18-40 with stable diopters for at least 1 year.
  • Myopia up to -6 diopters, myopic astigmatism up to -4 diopters.
  • Thin cornea that does not allow flap procedure (LASIK) or Smile PRO.
  • Low myopia where PRK is an excellent alternative.
CONTRAINDICATIONS

Who CANNOT Have PRK

  • Progressive eye diseases: keratoconus, glaucoma, cataract, macular degeneration.
  • Autoimmune conditions: diabetes, Sjogren syndrome, rheumatoid arthritis.
  • Severe uncontrolled dry eye.

PRK Surgery Steps

This is a complete ophthalmological consultation to evaluate eye health and determine the exact optical correction for best vision. Pupil dilation drops are necessary for accurate diopter measurement and complete examination. This includes detailed explanation of the upcoming procedure as well as your case particularities.

You will get answers to all your questions, so you know what to expect both during the procedure and post-operatively.

The complete pre-operative investigation and measurement package is performed in the clinic with state-of-the-art equipment. These are necessary for precise determination of technical parameters for the lasers, all based on your eye anatomy.

For convenience, you can do all investigations immediately after consultation, just allow approximately 2 hours for both.

On surgery day, the corneal epithelium is removed to expose the corneal surface. The excimer laser then directly reshapes the corneal layer in approximately 5-30 seconds per eye. The procedure is painless, using local anesthesia with drops.

At the end of the procedure, a therapeutic contact lens is applied to protect the eye and facilitate epithelium regeneration over the next 4-7 days.

Recovery after PRK requires 4-7 days for epithelium regeneration. During this period you may experience foreign body sensation, burning, tearing and light sensitivity. Antibiotic and anti-inflammatory drops are prescribed to support the healing process.

You will come for check-up the next day post-operatively and then for therapeutic lens removal (after 4-7 days), then at one month, 3 months, 6 months and one year, to monitor the healing process.

Frequently Asked Questions About PRK

PRK (PhotoRefractive Keratectomy) has specific indications. Unlike LASIK, no flap is created - the laser acts directly on the corneal surface, after epithelium removal. It is ideal for thin cornea, corneal scars or low myopia.

Recovery after PRK takes 4-7 days for corneal epithelium regeneration. During this period a therapeutic contact lens is worn and you may experience discomfort and light sensitivity. Vision stabilizes completely in 1-3 months.

PRK is recommended for people with thin cornea who are not eligible for LASIK/Smile PRO, myopia up to -6 diopters, myopic astigmatism up to -4 diopters, and age between 18-40 years with stable diopters. It is also indicated for people with low myopia who practice contact sports.

ATTENTION: Soft contact lens wear must be stopped at least one week before consultation and at least two weeks before surgery. For orthokeratology hard lenses, the term is at least one month.

Laser reshaping requires detailed and very precise measurements of all eye structures.

Yes, final PRK results are equivalent to LASIK/Smile PRO. The main difference is recovery time - PRK requires more time for healing, but the final result is just as good.

PRK procedures are considered among the safest in surgery. The procedure has a history of over 40 years and technology has advanced enormously, making it safer and more precise.

Although extremely rare, possible complications that may occur include: chronic dry eye, visual halos and haze, undercorrection, overcorrection, delayed epithelial healing, infection.

The vast majority of complications are treatable and usually resolve within the first months after surgery.

The goal of surgery is to see after the procedure without glasses, as well as you saw with (correctly prescribed) glasses before surgery. The corneal changes made during surgery are permanent.

As the eye is a living structure, it may undergo changes over time, independent of the procedure itself. These changes are related to the natural aging process of the eyes and the individual healing process. Diopter changes are usually minor and have no significant impact on vision.

Progressive eye diseases such as: keratoconus, diabetic retinopathy, glaucoma, macular degeneration, severe dry eye, cataract, eye inflammations prevent you from having laser vision correction surgery.

Degenerative or autoimmune conditions such as: diabetes, connective tissue diseases, Sjogren syndrome, rheumatoid arthritis, HIV infection affect post-operative healing and are contraindications for laser surgery.

Fear of surgery is understandable, but for laser procedures interventions are extremely fast and involve no discomfort. Although performed in an operating room with all protocols, they can hardly be called "surgical operations" in the sense known to everyone. Patients almost without exception say they did not realize when it was over and regret not having the procedure sooner.
